Please complete the form as detailed as possible. We will be using the information to determine winners throughout the season. Winners will be determined at random, as well as through the nomination information.
Open to Lakeland area kids under 18 years of age. Lakeland area includes Cold Lake, Bonnyville, St. Paul, Elk Point, Glendon, Ardmore, Fort Kent, Kehewin, Fishing Lake, Frog Lake, Saddle Lake, Ashmont, and rural areas in between that radius.
Please note: a condition of the prize is agreeing to have you and/or your child’s photo used on Lakeland Connect, its affiliate social media pages, marketing material and sponsors social media & marketing material.
A photo and/or video at the game of the winning child is required. Failure to provide the image will prevent you and your child from being eligible for any other Lakeland Connect and its affiliates and sponsors contesting & promotions.
